收集者及用途Who collects information and why
全家醫務中心／CareForAll 接收您透過所選診所聯絡渠道送出的資料，用於回覆查詢、處理預約、行政及服務意見，以及與有關查詢相關的醫療跟進。一般查詢不等於診症或已確認預約。CareForAll receives information you send through the selected clinic’s contact channel to answer enquiries, handle appointment requests, administration and service feedback, and arrange relevant medical follow-up. A general enquiry is not a consultation or a confirmed booking.
自願提供及所需資料Voluntary provision and necessary details
提供資料屬自願。請只提供與查詢相關及便於回覆的資料；資料不足可能令職員無法回覆或安排服務。一般查詢請勿傳送身份證號碼、完整保險卡資料、完整病歷或他人的私人資料。如需敏感醫療資料，請先向診所確認適當渠道。Providing information is voluntary. Supply only details relevant to your enquiry and a means of replying. Insufficient information may prevent a reply or service arrangement. Do not send identity numbers, full insurance-card details, complete medical records or another person’s private information in a general enquiry. Ask the clinic about an appropriate channel before supplying sensitive medical information.
誰會處理資料Who may handle the information
資料可由處理該事項的診所職員、相關醫護及協助通訊或行政的服務供應商處理；有法律要求或其他合法依據時，亦可能向相關機構披露。WhatsApp、電郵及網站託管服務有各自的資料處理安排；請參閱所選平台的私隱條款。網站由 Cloudflare 託管。Information may be handled by clinic staff responsible for the matter, relevant clinicians and providers supporting communications or administration. Disclosure may also occur where required by law or otherwise lawfully justified. WhatsApp, email and website-hosting services have their own data-handling arrangements; read the privacy terms of the platform you choose. The website is hosted by Cloudflare.
送出前與意見用途Before sending and use of feedback
本網站表格在瀏覽器整理訊息，沒有網站即時收件匣。開啟 WhatsApp 或電郵後，請核對內容，再由您自行按送出；通訊內容會交由所選平台及收件診所處理。服務意見用於跟進及改善服務，不會作公開病人推薦刊登。These forms prepare a message in your browser and do not provide an instant website inbox. Review the content in WhatsApp or email and press Send yourself. The selected platform and receiving clinic then handle the communication. Service feedback is used for follow-up and service improvement, not publication as patient endorsements.

Identity verification may be needed. Contact the clinic first rather than sending an identity-card copy through a general message.
